Transfer Request
We’re happy to keep you within the GreenTree family and find you a new home! The following form will give us a better understanding of how we can assist you. Please note that if you have a current balance owed, you must pay your balance before proceeding.
If you are fewer than nine months into your current lease, please contact us once nine months have passed since the start of your lease agreement. All transfers are subject to management approval.

Key Request
At GreenTree Property Management, we always strive to ensure the safety and security of our properties for the benefit of our Residents. In order to do so, we need to track/document keys. In an effort to keep sufficient records of who has access to our properties, please fill out the following information:
Lease Break
We hate to see our Residents go and would certainly like to know more about your intent to leave us! Filling out the following form will certainly give us a better understanding of how we can assist you. Please carefully note that lease break fees will be assessed in accordance with the terms and provisions of your lease.
Notice to Vacate
The following form is only applicable for Residents that fulfilled their 1-year lease term and are currently month-to-month.
If you are breaking your lease, please fill out the Lease Break Form instead of this form. If you have fulfilled your 1-year lease and intend to vacate the apartment, please fill out the following form:
Pet Application for Existing Residents
GreenTree Property Management is happy to welcome your new pet into your home! Please be sure that your pet meets our restriction guidelines found in our FAQ. Then, fill out the form on the Resident Portal Page by logging in here and clicking the PETS tab.
